This medical evaluation is the first step in determining whether ELLEvateHER Health services are appropriate for you. All care is provided through secure telemedicine by licensed healthcare providers.

Fee: $199 (one-time). This fee covers the medical evaluation only. Medications, laboratory testing, and ongoing care memberships are not included unless explicitly stated. If ongoing management is recommended, enrollment options will be presented separately.
What This Evaluation Does Not Include
- Ongoing care or management
- Follow-up visits
- Prescription refills
- Medication management
- Membership enrollment
Any ongoing care requires separate enrollment and consent.
After Your Evaluation
- You will receive written clinical recommendations.
- If ongoing care is appropriate, you may be invited to enroll in a care program.
- Enrollment is optional and requires a separate agreement and payment.
No further visits are scheduled as part of this evaluation.

By proceeding with this medical evaluation, you acknowledge and agree that:
- You are requesting a document-based medical evaluation via telemedicine.
- No live telehealth video visit is included as part of this initial evaluation.
- Recommendations and care guidance will be provided through the secure patient portal or email.
- Any invitation for further care (telehealth follow-up, membership programs, or prescription management) will be provided separately after review of your submitted medical information.
- You understand that prescriptions are not guaranteed as part of this evaluation.
- You consent to the use of telehealth technology for communication and care delivery when applicable.
- You understand that services are subject to state-specific regulations and licensing requirements.
- You agree to provide accurate and complete medical information to the best of your knowledge, including current medications, supplements, allergies, and medical history.
- You acknowledge that the evaluation and recommendations provided do not establish a traditional physician-patient relationship until you engage in further care (telehealth follow-up or membership program).
Payment & Refund Policy
Payment of the $199 medical evaluation fee is required before your intake is reviewed. Once the intake has been submitted and reviewed by a provider, the evaluation fee is non-refundable. This fee covers provider time and clinical review, regardless of treatment outcome.
