This medical evaluation is the first step in determining whether Amnicore Metabolic Health services may be appropriate for you. All care is provided through secure telemedicine by licensed healthcare providers.
What This Evaluation Includes
Your evaluation allows a licensed healthcare provider to review your health history and determine appropriate next steps.
Included in the evaluation:
• Comprehensive medical intake review
• Licensed provider clinical evaluation
• Written care recommendations when appropriate
Fee: $299 (one-time)
This fee covers the provider’s time and clinical review.
Laboratory testing, medications, and ongoing care programs are not included unless explicitly stated.tions will be presented separately.
Most patients receive their provider review and recommendations within 1–3 business days after completing the medical intake.

If you already have laboratory results and would simply like a physician interpretation before committing to ongoing care, you may prefer our Lab Insight service.
This service includes physician review of your laboratory results and written recommendations delivered through your secure patient portal.
How the Evaluation Process Works
Step 1 — Secure Payment
After completing payment, you will be directed to a secure online medical intake form.
Step 2 — Complete Your Medical Intake
You will provide information regarding:
• medical history
• medications and supplements
• symptoms and health concerns
• personal health goals
Accurate and complete information allows the provider to perform a thorough evaluation.
Step 3 — Provider Review
A licensed healthcare provider will review your submitted intake and determine whether Amnicore Metabolic Health services may be appropriate.
Step 4 — Clinical Recommendations
You will receive written clinical recommendations through the secure patient portal.
Recommendations may include:
• guidance following your evaluation
• additional testing if appropriate
• invitation to enroll in an ongoing care program
Enrollment in ongoing care is optional.
What This Evaluation Does Not Include
This initial evaluation does not include ongoing clinical care.
Specifically, the evaluation does not include:
• follow-up visits
• prescription refills
• medication management
• membership enrollment
Any ongoing care requires separate enrollment and consent.

Completion of this evaluation does not guarantee diagnosis, treatment, or prescription of medication.
All medical decisions are made solely by the evaluating provider based on:
• clinical judgment
• medical history
• current medical guidelines
Not all patients will qualify for treatment.

Payment & Refund Policy
Payment of the $299 evaluation fee is required before your intake is reviewed.
Once the intake has been submitted and/or reviewed by a provider, the evaluation fee becomes non-refundable, as it covers provider time and clinical review regardless of treatment outcome.
