ELLEvateHER Health delivers individualized medical care through a secure telemedicine process—designed to be thorough, efficient, and patient-centered.
Step 1: Secure Payment & Access to Medical Intake
To begin, you will complete payment for your Initial Medical Evaluation.
Once payment is submitted, you will receive access to our secure online intake form.
The intake includes your medical history, current medications, health goals, and relevant lifestyle factors. This information is required for a licensed provider to determine whether ELLEvateHER Health services may be appropriate for you.
Payment is required before a medical evaluation can begin and does not guarantee approval or treatment.
Step 2: Licensed Provider Intake Review
A licensed healthcare provider reviews your completed intake to assess medical eligibility, safety considerations, and whether care through ELLEvateHER Health is clinically appropriate.
Not all patients will qualify for treatment or prescription therapy. All determinations are made based on medical judgment and clinical guidelines.
Step 3: Written Clinical Determination & Recommendations
Following intake review, you will receive a written clinical determination through your secure patient portal.
This may include:
- Approval or non-approval for care
- Initial medical recommendations
- Identification of appropriate care pathways (e.g., metabolic care, GLP-1 program, further evaluation)
- Whether ongoing membership enrollment is required to proceed
A live visit is not automatically scheduled at this stage.
Step 4: Membership Enrollment (If Approved)
If you are approved for ongoing care, you will be invited to enroll in the appropriate membership or care program.
Membership enrollment is required before:
- Prescription management
- Ongoing provider oversight
- Telemedicine visits (when indicated)
- Continued clinical follow-up
Enrollment is optional if you are not approved or choose not to proceed.
Step 5: Telemedicine Visit & Individualized Care Plan
Once enrolled, and when clinically required, you will be scheduled for a secure telemedicine visit with a licensed provider.
During this phase, your provider may:
- Review symptoms and history in more detail
- Discuss lab work (if applicable)
- Confirm or adjust treatment strategy
- Finalize your individualized care plan
Telemedicine visit requirements vary based on your state and medical needs.
Step 6: Ongoing Follow-Up & Support
Safe and effective care requires ongoing monitoring. Depending on your program, follow-up may include:
- Periodic clinical check-ins
- Progress monitoring
- Medication dose adjustments (when prescribed)
- Secure messaging with the care team
Your provider remains involved to ensure care continues to align with your health goals and clinical needs.
